How to apply for a medical evaluation and financial assistance from the San Diego Sports Medicine Foundation.

Who can apply?
The SDSMF accepts applications from young athletes (ages 10 to 18) that have a sports related injury which requires further medical evaluation and the child does not have medical insurance or limited financial means to receive costly medical attention.

When to apply?
The SDSMF accepts candidate applications year-round.

Where to apply?
Please submit the SDSMF Candidate Application and required documents via postal mail to:
